To order tickets, please fill out the form below and return it and the payment to school by Monday January 25^th. Please write Boys Cosmic Bowling on the envelope. A confirmation e-mail will be sent if an e-mail address is provided. There is maximum allowance of 180 people at this event and spots will be reserved on a first come first serve basis. In the event we exceed this number an alternative date/time will be made.

The lanes will be pre-assigned and we can put 5 to 6 people on a lane and a pair of lanes accommodates 10 to 12 people. If you want to bowl on the same lane or pair of lanes with other people, please enclose everyone’s order forms and checks in one envelope. We will do our best to reserve lanes with or near your requests. If you indicate that you need bumpers all bowlers on those lanes will have to use bumpers.
